ABSTRACT

Objective @#To explore the etiology, clinical manifestations, diagnosis and treatment of multiple idiopathic root resorption to provide a reference for clinical diagnosis and treatment. @*Methods@# The clinical data of a case of multiple idiopathic root resorption were analyzed retrospectively, and the related literature was reviewed.@*Results@#The patient had no history of orthodontic correction, occlusal trauma, trauma history or other causes of root resorption. Clinical examination revealed full-mouth gingival congestion, redness, a loose texture, and variable degrees of destruction of the alveolar bone. Imaging examination showed that teeth 13, 16, 26, 36, 46 had idiopathic root resorption. The diagnoses were multiple idiopathic root resorption and periodontitis. The pathology tests showed that a large number of osteoclasts were present in the soft tissue surrounding the teeth. Whole-exome sequencing showed that there was a strong correlation between gene mutations (WNT7a and HSPG2) and the present phenotype. Root resorption of teeth without periodontitis was stopped after periodontal treatment during the 19-month follow-up. Tooth 13 was removed, and extraction socket preservation was performed. The etiology of idiopathic root resorption may be related to gene mutations, but it is not clear. At present, there is no effective treatment. @* Conclusion @#Multiple idiopathic root resorption has an unknown etiology, but it may be related to WNT7A and HSPG2 gene mutations. The rate of root resorption can be slowed by controlling periodontal inflammation.

ABSTRACT

Objective: to evaluate the evidence of the efficacy of the adjuvant use of laser in the surgical treatment of grade II furcation lesions and to demonstrate the clinical importance of the findings for decision making. Materials and methods: a manual bibliographic search was carried out where the following databases were consulted: PORTAL TIMBO FOCO, PUBMED, SciELO, BVS and GOOGLE SCHOOL. This search was restricted to the last 15 years and the selected articles are in English. Results: 105 articles that addressed the subject were selected and the search was completed by searching and reading the referenced bibliography in those articles. Conclusions: Low-power laser, both for biostimulation and as an anti-inflammatory has so far only shown positive short-term results as an adjunct to regenerative periodontal treatment in grade II furcation lesions. However, its long-term results are not clear due to methodological weaknesses and the insufficient number of existing studies. On the other hand, photodynamic therapy could be an alternative to control bone loss in furcation lesions caused by periodontitis. However, more studies are needed to elucidate the action of photodynamic therapy and photobiomodulation as adjuvants in the treatment of furcation lesions.

ABSTRACT

ABSTRACT: Diabetes mellitus and periodontal disease are pathologies with a high prevalence in the population, and their frequency is increasing. Both present common mechanisms of progression and manifest themselves in terms of comorbidity. The aim of this review was to explain the mechanisms of two-way association between diabetes mellitus and periodontal disease. For this purpose, a systematic search of the information in PubMed databases wasdeveloped, ScienceDirect, Scoopus and EBSCO of the last three years and selected those in English that included the pathways of association between diabetes mellitus and periodontal disease or viceversa in a unidirectional or bidirectional manner. Diabetes mellitus corresponds to a metabolic disorder, which is characterized by a hyperglycemia product of the defect in the production or action of insulin, or both. In recent times, emphasis has been placed on the influence of the periodontal condition on the glycemic control of diabetic patients. This review concluded that there is a bidirectional association of diabetes mellitus and periodontal disease, as well as the existence of limitations of studies to explain this relationship in terms of causality.

ABSTRACT

Aim: To compare the periodontal health of pregnant and non-pregnant women by applying the Community Periodontal Index (CPI). Materials and methods: In an observational, cross-sec- tional study, 100 pregnant women (PREG) and 50 non-preg- nant women (NonPREG) were recruited at the Dr. Raúl F. Lu- cini Provincial Maternity Hospital in Córdoba City. The CPI was determined in the 6 sextants of the mouth using a WHO 621 periodontal probe. The data were analyzed with Infostat SP software. P <0.05 was considered significant. Results: 70% of the patients were 18 to 25 years old. In the PREG group, CPI Code 3 was the most frequent, present in 240 sextants (40.1%), while in the non-PREG group, CPI Code 2 was the most frequent, with 39 sextants (43%). Treat- ment needs in both study groups are oral hygiene instruction, supra- and/or subgingival instrumentation, and/or correction of plaque retentive margins. Conclusions: Code 3 was the most frequent among preg- nant women, which corresponded to Periodontal Treatment Code (CTP) 2. CPI 1 and 2 were the most frequent in non-pregnant women, corresponding to CTP 1 and 2. This periodontal clinical condition can be treated with initial dental hygiene therapy, which can be performed by general dentists (AU)

ABSTRACT

ABSTRACT: Drug-influenced gingival enlargement is considered to be an important side effect related to the use of some medications and often produces important esthetic changes for patients, as well as clinical symptoms such as pain, bleeding, abnormal tooth movement and occlusion problems. Anticonvulsants, immunosuppressants and antihypertensive drugs have been reported as the main inducers of these periodontal disorders. This case report describes the 4-year clinical follow-up of a young patient with a history of excessive gingival enlargement as a consequence of drug therapy for epilepsy (phenytoin and phenobarbital); the nonsurgical periodontal treatment of the excessive gingival enlargement was associated with the replacement of the patient's epilepsy medications with valproic acid. We conclude that intensive mechanical control of bacterial biofilms, instruction on oral hygiene guidelines and behaviors and the substitution of alternative anticonvulsant medications are the best way to control the drug-influenced gingival enlargement.

ABSTRACT

Objective@#The use and effect of nitrous oxide sedation techniques in oral clinics were analyzed retrospectively.@*Methods@#Patients who were treated with nitrous oxide inhalation sedation in the clinic of the Affiliated Stomatological Hospital of Guangzhou Medical University from January 1, 2016, to December 30, 2018, were examined. Age and gender of the patients, dental treatments involved, reasons for nitrous oxide inhalation, sedative effects and adverse reactions were compiled.@* Results @#A total of 1 429 cases were examined, comprising 587 males and 842 females, and the average age was 32.64±16.34 years old. Among the patients who underwent nitrous oxide inhalation sedation in the oral clinic, 79.98% needed tooth extraction, and 79.50% had a dental fear of procedures. The patients were divided into following 7 age groups: 5-15 years old, 16-25 years old, 26-35 years old, 36-45 years old, 46-55 years old, 56-65 years old and > 65 years old. The sedation satisfaction rate of the 5-15-year-old group was 45.71%, and the sedation satisfaction rate of the other 6 age groups was 90.83%- 96.20% (P < 0.001). The incidence of total adverse reactions was 5.39%; the incidence was higher in females than in males, and the incidence was higher in the 16-25 age group than in the other age groups (P < 0.05). The most frequent adverse reaction was vertigo (81.82%).@*Conclusion@# Among the four common oral outpatient treatment programs including the extraction of teeth, dental implants, pulp treatment and periodontal treatment, patients undergoing inhalation sedation of nitrous oxide in the dental extraction most. The most common reason for requiring sedation is dental fear, and the sedative effect of the 5-15-year-old group was significantly worse than that of the other age groups. The incidence of adverse reactions of nitrous oxide sedation was low and manageable.

ABSTRACT

The objective of this study is to show the distribution of periodontal disease, risk factors, and importance of primary healthcare, for the improvement of clinical parameters. Two phases study transversal and nonrandomized trial (before - after), with educational intervention and conservative treatment, were carried out at Dental School of the Universidad Autónoma de Guerrero México, in 161 subjects who met the inclusion criteria. Oral healthcare education was carried out as well as conservative periodontal treatment, with six months follow up. Clinical measurements were performed with a Williams probe, O'Leary plaque index, calculus index and dental mobility Miller method. Periodontal disease was found on 82 % of all participants. Multinomial logistic regression analysis showed an odds ratio of 14.3 (95 % CI: 2.5, 82.1), 13.4 (95 % CI: 1.7, 103.5), 29.2 (95 % CI: 3.2, 260.9) and 68 (95 % CI: 6.6, 711.0) plaque in gingivitis, mild, moderate and severe chronic periodontitis, respectively. The longitudinal panel data analysis showed a significant effect (p <0.001) in the mean decrease of the clinical parameters after intervention, probing depth 0.4 mm (95 % CI: -0.5, -0.3), pockets depth 1.1 mm (95 % CI: -1.3, -0.9), amount of pockets 4.5 (95 % CI: -5.2, -3.7), bleeding 5.2 (95 % CI: -5.9, -4.5) and dental mobility 0.6 degrees (95 % CI: -0.7, -0.5). Primary healthcare is still the best option to improve the periodontal health in population who do not have access to specialty services. Dentists can achieve significant clinical improvement at very low cost, if they are aware of primary health care.

ABSTRACT

Objectives: To evaluate the type of "periodontal treatment" performed by general dental practitioners and the referral patterns of periodontists in Iraq. Material and methods: A total of 201 general dentists were asked to complete a self-administered questionnaire consisting of nine questions regarding periodontal treatment and the referral patterns of periodontists. Results: The study showed that 91.1% of general dental practitioners (GDPs) performed "phase-I therapy", and most of them done "scaling". Regarding surgical periodontal therapy, only 12.9% of them performed surgical periodontal therapy, half of the GDPs did gingivectomy, and less than 30% performed crown lengthening. When evaluating maintenance therapy after periodontal treatment, it was found that 77.9% of the GDPs scheduled appointments for patients in the maintenance phase, 49.4% of them after one month, 24.7% after three months, and 15.6% after six months. Using TRUF analysis, most general dentists believed that the patient and periodontal factors were responsible for the recurrence of periodontal disease. Conclusions: There is still a lack of awareness of periodontal surgical procedures among the "general dental practitioners". Thus, it is essential to increase the comprehension of periodontal treatment among general dentists. (AU)

ABSTRACT

Objective @#The purpose of this study was to investigate the relevant social and environmental factors affecting the occurrence of periodontal diseases during pregnancy in pregnant women and to analyze the influence of the periodontal status of women in the second trimester of pregnancy on small for gestational age (SGA) delivery.@*Methods@# A total of 215 pregnant women were enrolled in this study in the Department of Periodontology of the West China Hospital of Stomatology of Sichuan University from May 2015 to May 2018. Periodontal parameters, such as bleeding on probing (BOP), probing depth (PD) and clinical attachment loss (CAL), were recorded at 16-24 weeks of gestational age. Subjects were divided into the periodontitis (n=32) group, gingivitis (n=171) group and periodontally healthy (n=12) group according to their periodontal conditions. With the patient′s informed consent, the patient decided whether to receive periodontal treatment. Basic and socioeconomic information was collected through questionnaires. After delivery, subjects were divided into the SGA group and non-SGA group according to their birth results. The periodontal clinical indicators, questionnaire results and delivery results were compared among the groups.@*Results @#The mean PD (P=0.005, r=-0.192) and BOP% (P=0.003, r=-0.199) were negatively correlated with economic income. The family income in the periodontitis group was significantly lower than that in the healthy group and the gingivitis group (P < 0.05). The flossing use rate was significantly higher in the healthy group than that in the gingivitis group (P < 0.05). A total of 106 pregnant women received scaling and root planing, while 109 patients only received oral hygiene instruction. After delivery, SGA occurred in 23 cases (10.7%), and there were no significant difference in SGA incidence among the three groups (P > 0.05). PD ≥ 5 mm% and PD ≥ 4 mm% (P < 0.05) were significantly higher in the SGA group than in the non-SGA group. There was no significant difference in SGA incidence between the treated group and the untreated group (P > 0.05).@*Conclusion@#Family income and dental flossing use have an impact on the incidence of periodontal diseases during pregnancy. The severity of periodontitis in pregnant women is correlated with the incidence of SGA.

ABSTRACT

Mechanical instrumentation during periodontal treatment brings the formation of microcrystalline debris that inhibits tissue adhesion to the root surface and favors bacterial proliferation, which harms the results of the short and long term periodontal treatment. Nowadays, root conditioning with the use of biomodifi cators is an additional treatment option in the treatment of periodontitis and root coverage therapy. The aim of the present study is to conduct a literature review about the applications and the root conditioning with citric acid, tetracycline, EDTA and laser in the non surgical and surgical periodontal treatment (AU)

ABSTRACT

ABSTRACT

Abstract Objective: This study aims to evaluate the clinical and biochemical (oxidative stress and pro-inflammatory mediators) effects of the gaseous ozone use accompanied by scaling and root planning (SRP) in periodontal treatment. Material and Methods: The study population consisted of 40 patients with chronic periodontitis (CP) randomly sorted into two groups of 20. The experimental group received SRP plus 3 watts gaseous ozone in two separate applications five days apart, whereas the control group received SRP plus placebo. Clinical periodontal parameters were assayed and saliva samples were taken before the initial and one month after the second treatment. Periodontal examination assessed plaque index (PI), gingival index (GI), probing depth, and clinical attachment level (CAL). Total antioxidant status (TAS), total oxidant status (TOS), nitric oxide (NO), 8-hydroxy-2'-deoxyguanosine (8-OHdG), myeloperoxidase (MPO), glutathione (GSH), malondialdehyde (MDA), and transforming growth factor-beta (TGF-β) levels were evaluated from saliva samples. Results: Changes following treatment in PI, GI, probing depth, and CAL scores were similar for both groups (p>0.05). Of note, TGF-β levels were observed to be higher in the treatment group than in controls (p<0.05). Changes in 8-OHdG, TAS, TOS, NO, MPO, GSH and MDA levels, however, were not significantly different between groups (p>0.05). Conclusion: The findings of this study indicate that SRP plus gaseous ozone versus SRP alone does not correlate to a significant improvement in periodontal recovery.

ABSTRACT

Objective@#To evaluate the effects of periodontal therapy on the quality of life in chronic obstructive pulmonary disease (COPD) patients with chronic periodontitis (CP).@*Methods@#In a randomized controlled pilot study, 60 COPD patients with CP were randomly assigned into three groups to receive scaling and root planing (SRP) treatment, supragingival scaling treatment and oral hygiene instructions only with no periodontal treatment, respectively. The scores of each patient′s quality of life at baseline, 1 year and 2 years, respectively, were evaluated by using the standardized St George′s respiratory questionnaire (SGRQ). The SGRQ was composed of three dimensions: symptoms, activity, and impact. A score was calculated for each section and a total summary score was also calculated. A high score indicated poor health and a decrease in the score indicates an improvement in quality of life.@*Results@#There were no statistically significant differences among the three groups for age, gender, body mass index, smoking status, lung funtion, periodontal parameters and the SGRQ scores (including total, symptoms, activity and impacts scores) among three groups at baseline (all P>0.05). The SGRQ scores were all significantly correlated with major lung function parameters (r=-0.54 --0.28). The result of ANCOVA analysis adjusted for the influence of baseline measurements showed that the total scores of two periodontal treatment groups were significantly lower (P=0.01) than that of control group at 2-year follow-up (SRP group: 31.1±12.1; scaling group: 28.9±9.8; control group: 46.5±24.9). The symptoms score of SRP group (45.4±19.7) was significantly lower (P=0.03) than that of control group (53.6±25.4) at 2-year follow-up. The impacts scores of two treatment groups were significantly lower (P=0.02) than that of control group at 2-year follow-up (SRP group: 15.2±8.3; scaling group: 14.9±7.7; control group: 34.8±18.8).@*Conclusions@#Our preliminary results from this study suggest that non-surgical periodontal treatment in COPD patients with CP might improve the quality of life of the COPD patient.

ABSTRACT

@# Preterm low birth weight newborns are prone to complications during growth and development, such as growth retardation and infectious diseases, and the mortality rate of high-risk infants is higher than that of healthy newborns. Epidemiological data show that periodontal disease in pregnant women is correlated with preterm low birth weight. Periodontal disease in pregnant women is one of the risk factors for adverse pregnancy outcomes, such as  preterm   low birth weight. This paper addresses the correlation between maternal periodontal disease and  preterm  low birth weight, the influence of periodontal treatment on the incidence of preterm  low birth weight, and premature birth caused by maternal periodontal disease. The mechanism of infant weight is reviewed. The results of this review show that the current studies confirm the correlation between maternal periodontal disease and  preterm low birth weight. However, the causal relationship between maternal periodontal disease and preterm  low birth weight is not fully elucidated, and the specific mechanism and approach are not clear. A possible mechanism is that periodontal pathogens in pregnant women pass through the placental barrier to influence pregnancy outcomes. Therefore, more in-depth research is needed on the specific mechanism, approach, intervention and treatment based on the relationship between these two factors.

ABSTRACT

PURPOSE: The purpose of this study was to evaluate the prognostic effect of patient compliance with supportive periodontal treatment (PC-SPT). Chronic periodontitis patients were classified based on their compliance level, and factors affecting PC-SPT and the prognosis of PC-SPT were investigated. METHODS: This study selected 206 patients who started SPT after receiving periodontal treatment between 2010 and 2012. Patients who continued SPT through February 2016 were included. The patients were classified according to whether they exhibited complete compliance (100% of visits), excellent compliance (≥70% of visits), incomplete compliance (<70% of visits), or non-compliance (only 2 visits). Patient characteristics that could affect PC-SPT, such as age, sex, distance of the clinic from their residence, implantation, and periodontal treatment, were investigated. The number of newly decayed and extracted teeth, alveolar bone level changes around the teeth and implants, and implant removal were examined to evaluate the prognosis of PC-SPT. RESULTS: Sex and the presence of an implant significantly affected PC-SPT. Additionally, the number of newly decayed and extracted teeth and changes in alveolar bone levels around the teeth and implants were significant prognostic factors related to PC-SPT. CONCLUSIONS: PC-SPT in chronic periodontitis patients will help maintain periodontal health and prevent further periodontal disease.

ABSTRACT

With the purpose of determining the periodontal health status in adults patients of who receive dental attention in to the Dental School clinics of the Autonomous University of Chihuahua, 408 patients were reviewed and categorized by groups of sex, age, level of education and place of residence, evaluating the depth of the gingival grooves in diff erent gingival areas, classifying them according to the codes described by the community periodontal index of treatment needs (CPITN), which determines the complexity of the gross alterations in periodontal tissues according to 5 levels or codes. The results showed high needs of periodontal treatment by diff erent degrees in the population, up to 96%, while 4% required only preventive treatment and instruction, coinciding with similar studies carried out by other authors. It was also noted that the sex and place of residence did not infl uence the prevalence of periodontal alterations, on the other hand the need of periodontal treatment in the various age groups, is expressed only in the highest degree expressed as code 4 (C4), with an upward trend of need of periodontal treatment related to the upward trend of the diff erent ages. Higher frequency of periodontal alterations evidenced in people with middle education degree with a 34%. Concluding that it is necessary to encourage the culture of the self-care to the patient as a counterweight element, because is there the most eff ective prevention impact achieves (AU)

ABSTRACT

Objective: Gingivitis is caused by a mixture of Gram-positive and Gram-negative species and is characterized by inflammatory exudate in the marginal region. Probiotics have been introduced in the prevention and treatment of periodontal diseases and represent an innovative approach to maintaining oral health using beneficial bacteria when administered in adequate amounts. The mechanism of action of probiotics in the oral cavity is not fully understood but is commonly explained by a combination of local and systemic immunomodulation. The results of the studies suggest that probiotics may be useful in the prevention and treatment of gingivitis, but their effectiveness is still very questionable. Thus, more in vivo research is needed through randomized controlled studies evaluating the main periodontal clinical parameters, levels of inflammatory mediators and microbiological analyzes, especially to elucidate some still unknown mechanisms of action and to ensure the use of probiotics as an effective therapy in the prevention and treatment of gingivitis.

ABSTRACT

(AU)

Probiotics consist on living organisms which, when administered in adequate amounts, confer health and benefits to the host. The aim of this study was to evaluate the clinical influence of the use of probiotics as adjuvants in the treatment of Chronic Periodontitis (CP). A search conducted on PubMed and the results analyzed and 7studies were selected and their data presented through a table, which describes the results between the test group: scaling and root planning (SRP) associated to the use of probiotic and control group. Periodontal parameters including gingival index (GI), plaque index (PI), gingival bleeding index (GBI), clinical attachment level (CAL), periodontal probing depth (PPD) and bleeding on probing (BOP) have shown favorable results in both groups at the end of periodontal therapy, however, in some studies no significant differences were found between groups that justified the use of the probiotic. Authors carried out their researches administering different probiotic species, based on their properties of immune response regulation, inflammation control, direct and indirect actions on periodontal pathogens. Lactobacillus reuteri showed better clinical results compared to the other probiotics − Lactobacillus rhamnosus, Streptococcus oralis KJ3, Streptococcus uberis KJ2, and Streptococcus rattus− in all the presented results. The use of probiotics is a current issue in periodontics and there is still no consensus among researchers on its administration. New studies seem necessary for better understanding on the action of these microorganisms as adjuvants to periodontal treatment. (AU)

ABSTRACT

Objective; To analyze the clinical manifestion and treatment method of one patient with severe periodontitis complicated with oral lichen planus, and to clarify the effect of combined treatment method and the importance of regular follow-up. Methods; The clinical manifestion of the patient was that the bilateral moral teeth had become looseness and the gum had bled for six months, and the patients had a history of oral lichen planus in gum for 2 years old. The probing depth (PD) of all the teeth was 5-6 mm and the attachment loss (AL) was 8-9 mm, with II °-IE "mobility and bleeding on probing (BOP) (+). The teeth was treated with the regular basis periodontal treatment combined with gingival triamcinolone acetonide injection treatment. When the inflammation situation remained stable, the delayed implantation was performed and the crown restoration treatment was done for 26, 27, 36 and 37. The splinting treatment was done for 31 and 41 II mobile teeth. The regular follow-up periods were 1 week, 2 weeks, 1 month, 3 months, and 6 months after treatment during periodontal maintenance. Results; After combined treatment and regular follow-up, the PD of both anterior and premolar were 4 mm, and the BOP was negative. The clinical symptom of oral lichen planus of the patient was relieved, and there were no symptoms of congestion and erosion on the gum. After the inflammation situation was controlled, the chewing function of the left molar region was restored by implantation rehabilitation. A 3-month-follow-up cycle and excellent treatment compliance were beneficial for the control of disease. The optimal follow-up period of gingival condition maintained best was once per 2 weeks. Conclusion: The combined treatment can relieve the clinical symptoms of the patient with severe periodontitis complicated with oral lichen planus. Regular follow-up and excellent treatment compliance are conducive to controlling the condition of disease.
